What we Require from You
All participants are required to complete a Medical / Participation Information Form prior to taking part in one of our activities. For anyone under the age of 18 years, this is part of our Parental Consent Form. For anyone over the age of 18 we ask you to complete our Medical/Participation Form. All information is treated in the strictest confidence.
We need to know in advance of any medical or other issues that may affect your participation and enjoyment in our activities – that way we can, if necessary, adapt the activity or make appropriate provision.
We also ask that you take careful note of the Equipment List and what to bring/not to bring.
Please do not hesitate to contact us through the form below or on 07903 573585 if you have any questions about this.
